Login with your Member ID to enroll a dog or cat in Emergency Contact Data.

The Emergency Contact Data Pet Module lets you create a comprehensive electronic health record just like your own. Only pet dogs and cats are covered. A separate Pet Member ID is issued for each pet at sign-up. Upon enrollment, a “Welcome Packet” will be sent to you with the pet’s Member ID Card and three ID Tags.

The enrollment fee is $17.95 and renews each year on its anniversary date.
