The Emergency Contact Data Pet Module let’s you record important emergency contact and health information for your dogs and cats. Its content is customized for Dogs and Cats. A separate Pet Member ID is issued for each pet at sign-up; and emergency access to the pet’s contact and health data is by entering the Pet’s Name and Pet Member ID. Upon enrollment, a “Welcome Packet” will be sent to you with the pet’s Member ID Card and three ID Tags.

The enrollment fee is $17.95 for One Year or $2.06 per Month. Thereafter, membership renewal is on a one-year, recurring basis, billed automatically to your credit card.
